Clay (Hayden Christensen) experiences what is called "anesthesia awareness" during open heart surgery. In other words, he was awake, so he knew exactly what had happened, he could feel all the pain of the surgery, but the surgical team didn't notice it because his body was so paralyzed by the effects of the anesthesia that he died. He couldn't react to anything. Clay's young wife Sam (Jessica Alba) has to make important decisions during surgery, while Clay takes a strange journey in his own mind.
