John, a desperate and terminally ill patient, embarks on a journey to Mexico for a risky and experimental surgery, which takes place between the first and second films of the series. Little does he know that this surgery, supposedly for the treatment of his cancer, is a complete scam, and he has been deceived at his most vulnerable moment. The infamous serial killer returns to his best skill with a whole new purpose: to test the fraudsters who have fooled him with unique, insane, and masterfully crafted traps.
